An introduction to online booking:

In order to better our offering to you and to avoid lengthy waits on the phone, we provide online-booking for appointments. These bookings are not requests for appointments but rather a real-time booking.

To ensure correct booking, please note that you MUST read the instructions corresponding to the type of appointment you are trying to book prior to booking the appointment. You are responsible for booking your appointments in agreement with the rules hereby described. Appointments that are intentionally or unintentionally booked incorrectly may result in the loss of your appointment-making privileges and fees may be incurred on your children’s accounts.

THERAPISTS

Medicare does not cover services offered by the therapists. Throughout the booking process, you will be informed of the fees relating to the appointment you are booking. These fees are often covered by private insurances. Please consult your insurance company for details.

PHYSIOTHERAPY

  • Your child must be present for the appointment.
  • You must always make sure you are booking the appointment with the same physiotherapist. This also means that you are restricted to the physiotherapist's schedule (i.e. if you start with an evening appointment/ physiotherapist, you will have to continue having evening appointments with that physiotherapist). You cannot switch to a day-time physiotherapist if you started with a evening one, or vice versa.
  • The schedule for the different physiotherapists is as follows:

  • You are responsible for booking your appointments in agreement with the rules hereby described. Appointments that are intentionally or unintentionally booked incorrectly may result in the loss of your appointment-making privileges and fees may be incurred on your children’s accounts.

OCCUPATIONAL THERAPY

  • Your child must be present for the appointment.
  • The sessions are 1 hour.
  • You are responsible for booking your appointments in agreement with the rules hereby described. Appointments that are intentionally or unintentionally booked incorrectly may result in the loss of your appointment-making privileges and fees may be incurred on your children’s accounts.
  • If you are late, the therapist may refuse to see you. It is your responsibility to show up on time for your appointment. If you see that you may be late please call us and let us know. Please inform yourself to the best of your knowledge if there are any factors that may delay your arrival (road closures, constructions, events in the city, etc) and plan accordingly.  Don't forget events such as: Tour de l'Ile - happening every year at the beginning of June. SPEECH THERAPY

  • You need to fill out this form and return them filled out via email (info@clubtinytots.ca) or by fax (514-342-2911) before you can book an appointment.
  • The first appointment is always for an initial consultation. After the initial consult, your speech therapist will inform you what kind of appointment is needed next and the fees associated with that appointment.
  • Marie-Hélène Tcheuffa Kamo, speech therapist, offers her services in French while Meagan Honigman, speech therapist, offers her services in English.
  • Your child must be present for the appointment.
  • You are responsible for booking your appointments in agreement with the rules hereby described. Appointments that are intentionally or unintentionally booked incorrectly may result in the loss of your appointment-making privileges and fees may be incurred on your children’s accounts.
  • If you are late, the therapist may refuse to see you. It is your responsibility to show up on time for your appointment. If you see that you may be late please call us and let us know. PSYCHOLOGY

  • You need to fill out these forms  (Form 1 and Form 2) and return them filled out via email (info@clubtinytots.ca ) or by fax (514-342-2911) before you can book an appointment.
  • Sessions are 1h each
  • The FIRST appointment is with the parents alone (the child is not present at this visit). Both biological parents must be present; if one cannot come in, the other must bring a signed note acknowledging that the other parent is going to be attending the appointment alone. If the child is 14 years old or older, the child can come to the first visit with the parents.
