An introduction to online booking:

In order to better our offering to you and to avoid lengthy waits on the phone, we provide online-booking for appointments. These bookings are not requests for appointments but rather a real-time booking.

To ensure correct booking, please note that you MUST read the instructions corresponding to the type of appointment you are trying to book prior to booking the appointment. You are responsible for booking your appointments in agreement with the rules hereby described. Appointments that are intentionally or unintentionally booked incorrectly may result in the loss of your appointment-making privileges and fees may be incurred on your children’s accounts.

SICK VISIT: Please select the reason for your visit below

A sick visit is described by any new problem, sickness or new injury for which the patient has not previously seen this doctor.

Behavior problem, scholastic problems or ADHD.

  • This appointment must be booked with the child's REGULAR DOCTOR (“Check up doctor”).
  • To book this appointment you must have seen your regular doctor for a check up in the last 365 days. If you have not, book this appointment as a check up.
  • If you do not have a regular doctor and would like to have one, please call 514-342-9911 so that a receptionist may assign you one. The first visit with this doctor must be for a check up. The doctor needs to do a full assessment of your child in order to discuss such issues. Please see the instructions for booking a check up.
  • Only one problem can be dealt with at this appointment. If there are several issues that you would like to discuss with the doctor, please book a separate appointment.
  • Your child must be present for the appointment.
  • All test results or visits from elsewhere should be brought in to the appointment.
  • Whenever possible and to help the doctor to diagnose your child more rapidly and accurately, please bring a video that displays the behaviours for which you are consulting the doctor, or a symptom diary with the following information: date, time, exact description of symptoms, intensity/ severity, duration of episode, what triggered it and what made it go away.
  • Please bring the child's Medicare card as well as the blue vaccine booklet with you.
  • You are responsible for booking your appointments in agreement with the rules hereby described. Appointments that are intentionally or unintentionally booked incorrectly may result in the loss of your appointmen
  • making privileges and fees may be incurred on your children’s accounts.
  • If you are late, the doctor may refuse to see you. It is your responsibility to show up on time for your appointment. If you see that you may be late please call us and let us know. Rash, skin infection or other skin problem.

  • The appointment should be booked with the child's REGULAR DOCTOR whenever possible. If your regular doctor is not available and the issue isn't urgent, it is advised to wait until you can see your regular doctor.

  • If your regular doctor is not working but you would like to be seen as soon as possible, and this problem is urgent in nature, book with any doctor.
  • Only one problem can be dealt with at this appointment. If there are several issues for which you would like to consult the doctor, please book a separate appointment OR book a check up appointment with your regular doctor if you haven’t seen him/ her in more than a year. Pain related problem (e.g. arm pain, leg pain, etc)
